Without new legislation, Medicare Clinical Laboratory Fee Schedule (CLFS) rate cuts of up to 15% for approximately 800 laboratory tests are slated to take effect on January 1, 2026. Additionally, independent laboratories, hospital outreach laboratories, and large physician office laboratories (POLs) will be required to report some 2019 private-payer volume and payment data by March 31, 2026. The Centers for Medicare & Medicaid Services (CMS) will use this data to determine Medicare CLFS rates for 2027–2029.
