CPT^® code 81479 (unlisted molecular pathology procedure) should only be used for unique procedures that are not adequately addressed by any other CPT code. It should be reported only once per patient, per specimen and date of service. 81479 has one of the highest denial rates. This can be due to lack of documentation, failure to obtain pre-authorization, or incomplete claim forms.
